Our expertise in systems thinking underpins our philosophy on population health, and has produced outstanding results. Our multi level approach to healthcare systems translates to the world of population health in two main categories:

Overall, System - Wide Population Health

Population health is the understanding and maintenance of health in a population of individuals, such that they achieve the best health they are capable of by addressing all factors contributing to their health. This includes clinical, genetic, environmental, social, political and financial aspects among others. As such, our philosophy on creating an effective population health system revolves around the concepts of connectedness and knowledge. For people to build the network of supporting resources they need to address the individual issues in their lives, these resources need knowledge of each other's existence, capabilities and ability and willingness to work on truly integrating service delivery to the patient.

Knowledge is only one component though. The true integration starts to emerge when that knowledge is enhanced by a human to human relationship, as participants in the network begin to understand each other on a much deeper level. BR was a part of the Vermont Blueprint for Health's Care Coordination workgroup and contributed significantly to the establishment of that nascent population health framework in the early 2010's. We have followed on by regular contributions to the Accountable Community for Health framework in two VT HSA's, which have established local and statewide networks and relationships which are starting to yield excellent results. BR has developed unique tools for field evaluation of SDOH (Social Determinants of Health) utilizing the work and discoveries of the Camden Hot Spotting teams. We can offer these tools to population health workgroups and instruction on how to most effectively utilize them. 

Factor - Specific Population Health

A component of population health is the evaluation of the population for specific conditions. A common example is the control of Diabetes using A1c levels as a marker. We have worked with CAH's and clinics on developing Panel Management systems to identify and manage Diabetes, Obesity, Smoking and other factors as a regular part of their practice workflows. This has often been integrated with a Patient Centered Medical Home (PCMH) implementation. As these systems are implemented and mature, the state of conditions in the population is brought under control.

Running a medical practice is a complex endeavor. Sometimes it seems that just keeping the doors open and managing the patients coming through the door is about as much as you can do. Even then, it's highly likely that providers are taking work home and doing it there. It's common now for providers to be spending 4+ hours a night catching up on charts and other administrative tasks from the day's work. 

One of the basic premises of the Bryan Research Transformation Model is that of balance. The four major subsystems of an organization need to be in balance to enable alignment and satisfaction of demands by delivery of services and / or products, with purpose.

Sometimes, running a practice feels like everything is metaphorically on fire, there is no time for rest and it's not just a struggle to keep up with the constant demands, it seems impossible to keep up. In such an environment, how do you allocate time to work on improving the practice? 

We've been coaching practices for years now. All varieties of practices. One provider to 80 providers, in primary and specialty arenas. There is one common thread for every practice we've worked with. They never have enough time. 

One of the major problems is that this is a self perpetuating cycle. Intrinsic problems in the practice produce sub optimal results which require workarounds to manage, which takes up more time, which leaves no time to even think about solving the problems permanently and breaking the cycle

We have extensive experience in coaching practices. Starting with an initial assessment to gauge a current state, and identifying pain points and internal responsibilities, we begin to work on problems one by one according to self identified priority. We coach the providers and staff in managing these problems, and teaching them state of the art methods to solve the issues permanently. We coach the senior staff, providers and owners in different ways to manage, reinforcing the learning undertaken by the staff such that an army of problem solving resources is created and nurtured. As problems are solved, time is released. Time which used to be spent on wasteful activities, can now be redeployed to invest in other, more productive ways. 

This takes time, and careful coaching to engineer a cultural shift upon which a true transformation can be built.

Integrated Behavioral Health in Primary Care

Bryan Research was contracted in 2016 to provide Practice Coaching services on the University of Vermont's Integrated Behavioral Health in Primary Care (IBHPC) initiative.

This is a research study designed to evaluate the effectiveness of truly integrated behavioral health, as opposed to behavioral health that is delivered in a co located manner, without true integration (as defined at https://clinicaltrials.gov/ct2/show/NCT02868983). It is a multi year, multi state study, comprising 44 practices across the USA who are evaluated initially, taken through a defined intervention with coaching support and evaluated both within and at the conclusion of the process. Lee is one of the three primary coaches on the coaching team (supported by two secondary coaches). All coaching is conducted remotely via Zoom. The study is still ongoing at the time of writing (February 2020) so we cannot go into details at this time.

As a result of this deep exposure to a spectrum of practices undergoing this intervention, we have significant experience in the evolution of practices towards integrated behavioral health, and can offer that experience and expertise to practices in the general domain who wish to benefit. We can:

  • Evaluate your practice for degree of BH integration as a baseline
  • Design a customized intervention for your practice, to take it through a change pathway resulting in increased integration
  • Coach your practice remotely at practice - defined intervals to keep intervention tasks on schedule, train team members in QI tools and techniques, keep momentum and address road blocks discovered along the way
  • Design a practice wide transformation that incorporates and builds on the IBH intervention and aligns with the practice strategic goals.

We believe in the development and dissemination of knowledge in all its forms.

Our work over the last 30 years has allowed us to develop some unique insights into the world of work and business, most of which we incorporate into our daily work with clients. We understand the concepts are difficult to understand and visualize, and do our best to train clients on demand as they incorporate principles into redesigned workflows and quality initiatives. It is because of this we write books, articles and produce podcasts, courses and videos, providing access to the research in as many forms as possible so clients can utilize it as much and as easily as possible. 

We have two books in draft right now:

Managing your Practice by Value Streams is a handbook of around 120 pages designed to give all the knowledge you need to start thinking of your business in terms of levels and value streams. It takes the reader through the initial concepts, then describes optimal pathways for working on your practice to reflect the alignment on patient experience. Focussing on this above all produces some powerful effects. Higher patient satisfaction, reduced waste, reduced burnout and increasing the ability to experience joy in work again. We show you how to visualize workflows by using the patient flow as a backbone using a custom designed mapping technique derived from the original Toyota Production System, but adapted and improved substantially for use in healthcare environments. We show you how to start a transformation and how to engineer your culture with purpose to solidify the gains and achieve true change. This book is estimated for publication by April 2020 and will be available on electronic and paper formats. 

The Model is the result of 30 years of research in organizational design and improvement in multiple sectors. It describes the multi layered structure of organizations, how the layers nest together and how each layer interacts with those above and below it. Each layer has a corresponding set of business processes, each of which have their own Key Performance Indicators (KPI's). We describe a "starter set" of KPI's for you to begin your journey of advanced management of your business.  We describe the role of people at all levels in the business, taking influence from the works of Goldman and Spear, Herzberg and Maslow and constructing a model based upon a solid foundation of respect for people. We show how organizational design differs in traditional and systems thinking perspectives, and how to convert from one to the other. We show you how to manage the business, from all perspectives, so it can deliver the services and products demanded of it as closely to the point of need as possible. 

We will be publishing thought leadership articles regularly, both on this website and on social media platforms such as our LinkedIn and Facebook pages. Twitter is our immediate thought channel, and we will be tweeting very frequently and following many tweet feeds. We will be producing white papers on subjects related to the model and transformation processes and making those available on the website too.
